Dermal Fillers: What to Expect from an Experienced Injector
Patients often come to our practice focused on one specific area. It is natural to be most concerned with what you notice first in the mirror. But in our experience, it is rarely about just one thing.
A change in one area can shift the harmony of the entire face, and a skilled injector always evaluates the full picture before ever picking up a syringe. Guiding a patient toward what will actually serve their aesthetic goals is as important as the treatment itself.
At The Aesthetic Method, every appointment starts with a Full Face Assessment, and we often recommend a different approach than what the patient initially had in mind.
Lips are a good example. So many patients associate HA filler with lip filler that the two have become almost synonymous. And lips are a legitimate concern. But they rarely exist in isolation. The areas surrounding the mouth, including the nasolabial folds, the midface, and the chin, all shape the overall perception of the face.
Hyaluronic acid fillers restore lost volume, smooth deep folds, and refine facial contours. Used strategically, as part of a plan that also considers skin quality, they are a key tool in achieving natural, balanced results. The three cases below show what that looks like in practice.
What a Full-Face Approach Looks Like
Case 1: The support comes first
Adding volume to the lips and adding support to the surrounding areas. Injector: Lily Kim, RN, BSN
A patient came in wanting lip filler. After a Full Face Assessment, Lily Kim, RN, BSN, put together a treatment plan that included filler in the temples, midface, smile lines, aperture (the triangle area next to the nose), chin, chin shadow, and lips.
The patient could not complete all areas in one appointment due to budget and a vacation coming up. So the team focused on what mattered most to her: her lips. But to treat the lips properly, the surrounding areas needed support first. That meant the aperture, chin, and chin shadow.
Treating her lips alone would have done her a disservice and it would end up looking ducky and unnatural. That’s a non-negotiable in our medspa!
Case 2: Facial Balancing
Transformation 2 weeks after HA Filler injection (midcheek, lip, chin, and chin shadow) by Dana Hasselschwert, RN, BSN
After a Full Face Assessment, Dana Hasselschwert, RN, BSN, and her patient came up with a game plan together: midcheek, lip, chin, and chin shadow filler. Each area was selected based on how it contributes to the balance of the mid and lower face, and the result reflects that.
Case 3: Full Face filler
Before & After photos from a dermal filler patient of Stepanie Stewart, PA-C
A patient came in saying she needed chin filler. After a Full Face Assessment, we agreed that chin filler would be useful, but only as part of a more comprehensive plan.
We treated five areas: temples, midcheek taper, chin, pyriform, and under eyes. Five syringes total, a volume of about one teaspoon.
Here is why each area was necessary. Without temple filler, the natural cheeks would have looked unbalanced. Without midcheek and pyriform support, the under-eye filler would have looked tubular and needed to be dissolved. Treating the chin alone would have left the rest of the face unchanged, making the result look out of place rather than natural.
Nasolabial Folds: One of the First Signs of Aging
Nasolabial folds are the lines running from the sides of the nose to the corners of the mouth. For many women, they are one of the first visible signs of facial aging. Some notice them as early as their late twenties. They tend to deepen from the mid-thirties onward.
Nasolabial folds cannot be treated with skincare alone, but dermal fillers can restore volume directly to the fold and the surrounding structure.
Because this area moves constantly with facial expressions, product choice matters. A filler used here needs to integrate naturally into the tissue and move with the face.
At The Aesthetic Method, we use Evolysse™ for this concern. This hyaluronic acid filler is FDA-approved for moderate to severe nasolabial folds. We view its Cold-X Technology as the first major advancement in HA fillers in over a decade. Cold-X preserves the natural structure of the HA molecule during production, resulting in better tissue integration in dynamic areas.
What to Expect at Your Appointment
Before any filler treatment, your nurse injector — Lily, Dana, or Stephanie — will review your health history and discuss your concerns and goals to ensure the treatment plan is right for you.
If a filler such as Evolysse™ is selected as part of your plan, you can feel confident knowing the formula contains lidocaine to help make the injection experience more comfortable.
Results are visible immediately, with the final outcome settling over one to two weeks as the filler attracts water and integrates naturally into the tissue. Common side effects may include mild swelling, bruising, or tenderness at the injection site, which typically resolve within a week.
Your provider will review personalized aftercare instructions with you before you leave.
Is HA filler right for You?
HA filler is appropriate for most adults who have noticed volume loss, deepening folds, or loss of facial definition. At The Aesthetic Method, we review your full health history at your first consultation before making any recommendation. Certain conditions, including pregnancy, breastfeeding, a history of severe allergic reactions, or active skin infections in the treatment area, require a more detailed conversation before proceeding.
We will also tell you if filler is not the right approach for your concern. In some cases, addressing midface volume with filler or treating skin laxity with RF microneedling will do more than injecting directly into the fold. The Full Face Assessment is where we work that out together.
Key Takeaways
Volume loss happens across the whole face. Treating it well means looking at the full picture before deciding where and how much to inject. The cases above show what that looks like: a treatment plan built around the face, not a single area.
